Illness Benefit
· If you are out of work due to illness then you are entitled to Illness Benefit as part of your P.R.S.I taxes.
· Even if you think you might not get a payment, you should request a Cert as a submitted certificate entitles you to PRSI ‘stamps’ which are needed for State Pension on retirement.
· Non-COVID related Illness: You may apply for illness benefit after a 6 day wait period.
· COVID19 related Certification: If you have Covid19 or have to isolate because of Covid-19 contact or testing then you are entitled to an accelerated and enhanced Illness Benefit Payment payable from Day 1 of isolation. go to Department of Social Protection website and fill in their IB1 form or alternatively call to the surgery to collect form. You do not need to get a Medical certificate from your GP, you can upload your positive PCR or Antigen test.
STEP ONE
Request a Medical Certificate of Illness (Med 1 form) from your GP. The e-Cert is sent electronically to the Department.
* Certs will only be issued following an initial review by your GP.
**The E-Cert sent covers the totality of the illness and no ‘First, Middle or Final' cert is sent
STEP TWO
Go to Department of Social Protection website and fill in their IB1 form or alternatively collect one from Reception.
Blood Results
When you have a blood test carried out, your results may take up to 2 weeks to come back, do not phone the surgery enquiring about results before this period. If your result is abnormal the doctor who ordered the blood test will contact you by phone. If your results are normal you will receive a text message from the doctor.
UTI’S
If you think you may have a urinary tract infection, please ring the surgery at 9am to book a phone consultation with the doctor to discuss the results of dipstick carried out in our surgery. You will be asked to drop the sample into Reception before 10am to allow us to send the sample to the Lab to be analysed. Please note we are unable to send samples to the Lab on Fridays as we do not have a courier.
Driving Licence Appointments
Please ensure you book an appointment in advance of your licence expiring as this is not an urgent same day appointment you may have to wait up to 2 weeks for an appointment. Once you have your appointment made please book your appointment with NDLS within a month of your medical/eyesight exam with us. You can download form in the forms section . Please have all your details filled in prior to your appointment.
